I know that Spiro Dedes is one of the worst announcers in the history of sports, but sometimes one has to bear with it. To answer your questions:
Houston had successfully guarded A&M's 2 primary shooting options. The in-bounds pass had to go to someone OTHER than A&M's first and second options. As for not fouling, the announcers mentioned this at least a couple of times: Sampson doesn't believe in doing that. Some coaches do, some coaches don't. Apparently Sampson believes that, with one of the best defenses in the country, he doesn't need to foul in that situation.
It's not malpractice. It's just coaching decisions that do or don't work out.
